Abstract
Acute Care for Elders (ACE) is a model of care designed to improve functional outcomes and to improve the processes for the care of older patients. This model includes: an environment of care designed to promote improved function for older patients; an interdisciplinary team that works together to identify/address the vulnerabilities of the older patients; nursing care plans for prevention of disability; early planning to help prepare the patient to return home and a review of medical care to prevent iatrogenic illness.Acute Care for Elders: A Model for Interdisciplinary Care is an essential new resource aimed at assisting providers in developing and sustaining an ACE program. The interdisciplinary approach provides an introduction to the key vulnerabilities of older adults and defines the lessons learned from the Acute Care for Elders model. Expertly written chapters describe critical aspects of ACE: the interdisciplinary approach and the focus on function. The fundamental principles of ACE described in this book will further assist hospital leaders to develop, implement, sustain and disseminate the Acute Care for Elders model of care. Acute Care for Elders: A Model for Interdisciplinary Care is of great value to geriatricians, hospitalists, advance practice nurses, social workers and all others who provide high quality care to older patients

Abstract
The trend toward patient self-management (PSM) of chronic disease is accelerating at a rapid pace along with the evolution of home-based or mobile technologies to support this care. Yet the development of self-management practice standards and advanced practice nursing support has been haphazard. This book fills a glaring void by addressing, against a backdrop of current best practices in PSM, such questions as: What are appropriate standards of safety in PSM? How can we be assured those standards are met? How does one reach a good prognosis about whether or not patients will be able to practice PSM? What level of effectiveness and efficiency should PSM reach to be considered a good policy option? Grounded in a clear ethical practice framework for PSM regimens, the text discusses PSM of the major chronic diseases along with best practice intervention strategies. The text maps out the implementation of the PSM framework for both patient and institution, supported by numerous case examples. Also addressed are PSM challenges to Advanced Practice Nursing, three ethically valid assessment tools, and relevant health policy concerns.

Abstract
An Interdisciplinary Approach to Homecare Improve patient care through interdisciplinary teamwork Medicare is not concerned with outcomes specific to therapy, nursing, or social work. Since payments are based on the overall patient outcome, agencies need to encourage disciplines to work together to achieve the desired end results. Cindy Krafft, PT, MS, helps you develop a collaborative, interdisciplinary approach to patient care. You'll learn to make proper use of previously collected OASIS-C data to draft effective and thorough care plans that benefit your agency and its patients. Benefit from this book by: Ensuring collaborative patient care by communicating with members of the interdisciplinary team (IDT) despite infrequent face-to-face interactionsTaking advantage of information you already possess?such as OASIS-C documentation?to drive the care planning processEstablishing care conference best practices to make them effective and efficientDetermining medical necessity through an understanding of the viewpoint of other disciplinesInitiating appropriate referrals to other services as indicated Removing the task-oriented mind-set prevalent in agencies today and develop patient care plans with input from the entire IDT

Abstract
This book presents contributions from researchers and practitioners in the field of cybertherapy which not only illustrate the progress made in treating a variety of disorders, but also identify the challenges still faced in this field; such as the development of easy to use and more affordable hardware and software as well as the need to address potential side-effects and implement more controlled evaluation of cybertherapies as compared to more traditional treatments. The book, which will be of interest to health professionals and patients alike, is divided into four sections: Critical Reviews contains summaries and evaluations of emerging cyber therapy topics; Evaluation Studies includes chapters which undertake to solve some specific practical problems and assess the value of cybertherapy interventions; Original Research addresses new cybertherapy methods or approaches; finally, Clinical Observations explores case studies and research protocols with long-term potential.
